At our organization, we prioritize the highest standards of care for our members. To achieve this, all our Registered Nurses (RNs) undergo rigorous training, including the Uniform Assessment System for New York (UAS-NY) program mandated by the Department of Health (DOH) and state regulations. In addition to UAS-NY, our RNs also complete specialized Premier supplemental training, meticulously designed to accurately capture quality and risk scores specific to each member's needs.
This meticulous training process not only guarantees that we capture the highest allowable risk scores but also enhances the health plans overall quality ratings. Our commitment to excellence extends to every aspect of our services.
The Uniform Assessment System:
In 2013, the Office of Health Insurance Programs, Division of Long-Term Care, introduced the Uniform Assessment System for New York (UAS-NY). This comprehensive program serves as an integral part of the assessment system for in-home and community-based long-term care services and programs in the state.
Our Expertise:
Homebase Care is entrusted with providing delegated member assessment services, including enrollment, reassessment, and SCIC, on behalf of health plans across various programs such as Medicaid, PACE, MAP, FIDA (dual eligible products), and MLTC/MLTSS managed care programs.
To ensure the highest quality of care, we maintain a specialized team of RNs who have completed state-specific assessment training, in addition to Homebase Care's managed care and risk/quality score training. Complementing our dedicated RN field assessment staff is an in-house administrative team that provides vital support, scheduling, quality audit, IT, and managed care program submission and reporting services.
Our Approach:
Our RN assessors collaborate closely with health plan managers to develop personalized, person-centered care plans for each member. We are dedicated to delivering exceptional care tailored to individual needs.
